Job Description

Are you driven to provide a people centric service that breaks through barriers between local and national support services and activities? Can you enable individuals to identify personal outcomes to improve their health and wellbeing? Are you able to develop effective pathways to a range of service providers? This might just be the role for you!

The key to this role is in the support of people to live well through strengthening connections between community resources and primary care. This will be born through developing professional working relationships with GP practices and local health improvement teams as well as developing knowledge of other national and local service providers.

The team covers GP Practices in Lochgilphead, Campbeltown, Islay, Dunoon, Bute and Helensburgh.

We are looking to recruit a member of staff for surgeries in the Lorne area for x2 days/ 15 hours per week, the working pattern can be discussed at interview.

The Community Link Worker role is a ‘generalist’ role; meaning that you will see people facing all kinds of health and wellbeing challenges; including low level mental health difficulties, loneliness, disability, long term conditions, bereavement, poverty, relationship difficulties, experience of trauma, housing problems and parenting challenges to name just some.

Community Link Workers offer interventions to support and motivate people to improve their health and wellbeing; and link them with the right tools, community support and services. This is done using face to face, online, telephone and community appointments. Community Link Workers build up great connections and local presence in the community so as to strengthen the connection between the GP Practice and the community. It is an excellent time to join our organisation as a Community Link Worker.

We Are With You have been providing support services in Argyll and Bute since 2015, and we have strong connections and presence across the whole region.

 

You will have the ability to build and maintain professional working relationships to enable to the development of effective pathways to a range of service provisions. You will take responsibility for removing barriers to the access of relevant services, and for linking community resources and primary care. You will play a key role in practice team meetings to discuss caseloads, concerns, gaps in service and improvements to be made. You will also implement feedback on learning from the service outcomes and deliver alcohol brief interventions, contributing to Local Drug Partnership targets and HSCP targets.


Required Skills

  • SVQ level 3 Social Services and Healthcare or SVQ level 3 Community Development
  • For applicable services - registered with the Scottish Social Services Council (SSSC) or meets the requirements for registration
  • Knowledge and experience of working with people who are experiencing complex social and emotional circumstances
  • Experience in carrying out regular monitoring and evaluation to monitor outcomes and review practice in light of evaluation.
  • Extensive effective interpersonal skills in working with people on a 1:1 basis and with organisations, including the ability and sensitivity to work with vulnerable people to build trust which allows the sharing of personal information.
  • Ability to listen, motivate and support individuals and stakeholders.
  • Ability to develop a non-dependent relationship with awareness of personal and professional boundaries, whilst being supportive and respectful of others and non-judgmental.
  • IT skills including the use of Google Suite and ability to produce written reports and maintain ‘client’ records.

Desirable Skills

  • Skills in presenting information clearly and concisely to a variety of audiences.
  • Educated to degree level or equivalent within a relevant field e.g. community learning and development; social work or health.
  • Experience of working in partnership with other local and statutory organisations.
  • Experience of working within primary care
