Salary: £25,178 to £25,178 per pro_rata

Job Description

Are you driven to provide high quality support to people living in North Berwick (EH39) and Gullane (EH31) to improve their health and wellbeing? Are you able to make good connections and build a network with local support services? Do you want to make a lasting contribution to the health of people in these local communities? This might just be the role for you!

The role is based in GP Practices, where you will work as part of the team; providing 1:1 appointments to people who are referred to you by the GP and other Practice staff. You'll concentrate on supporting people with their non-medical needs; for example isolation, low level mental health difficulties, housing, money worries, bereavement, relationship difficulties and other common life events. This role contributes to the work of Primary Care by leaving the Practice team to concentrate on medical matters. 

Access to a vehicle is essential for this role.

 

The right skill set for this role includes active listening, action planning, motivational interviewing, advocacy and learning to use evidence based tools. The post holder will build detailed knowledge of local support services and organisations; and connect people with these to sustain better health in the longer term . The post holder will have the ability to forge strong relationships in a busy GP Practice, as well as with community organisations. In time, there will be the opportunity to develop small scale group work on health and wellbeing topics.

This role is within a small and enthusiastic team; with a supportive culture and great learning opportunities. This is a fixed term contract until 31 January 2024, the hours are 15 per week which can be discussed at interview. Driver with own car preferred to enable travel between GP Practises. 


Required Skills

  • SVQ level 3 Social Services and Healthcare or SVQ level 3 Community Development
  • Knowledge and experience of working with people who are experiencing complex social and emotional circumstances
  • Experience in carrying out regular monitoring and evaluation to monitor outcomes and review practice in light of evaluation.
  • Extensive effective interpersonal skills in working with people on a 1:1 basis and with organisations, including the ability and sensitivity to work with vulnerable people to build trust which allows the sharing of personal information.
  • Ability to listen, motivate and support individuals and stakeholders.
  • Ability to develop a non-dependent relationship with awareness of personal and professional boundaries, whilst being supportive and respectful of others and non-judgmental.
  • IT skills including the use of Google Suite and ability to produce written reports and maintain ‘client’ records.

Desirable Skills

  • Skills in presenting information clearly and concisely to a variety of audiences.
  • Experience of delivering small group work
  • Experience of working in partnership with other local and statutory organisations.
  • Experience of working within primary care
